FCYBL "Player Acceptance" Form
 
Overview
FCYBL Rule 5.1.2 allows clubs to have players to sign a form accepting their position on a team. If this form is completed and signed by the player and a parent, that player is then prohibited by FCBYL rules from trying out with or playing for another FCYBL team.
